MUST:
- Report to specified person prior to commencing work on site and attend the Site Safety Induction and any Tool Box talks deemed necessary by the contractor.
- Adhere to all Health and Safety Instructions given by management.
- Wear Personal Protective Equipment (helmet, safety boots & high viz vest) and any other protective equipment necessary in the correct manner to provide the protection intended.
- Observe all Site Health & Safety Rules.
- Use the correct equipment for the job and use only as intended. Ensure that the equipment is in good condition and all safety devices are in place.
- Report all damage and accidents without delay even if no injury is sustained.
- If you have any concerns about health & safety please speak to your Site Foreman.
- Carry your CSCS card on you at all times.
- Machine Drivers, Banksman, Scaffolders or other trades, must have relevant competence Tickets on their person at all times on site.
MUST NOT:
- Carry out any Tasks or work in any area where you feel unsafe.
- Use any tools or plant which you are not fully trained on or competent to use.
- Leave site during working hours without informing the Site Manager.
- Work alone outside normal working hours.
- Drive or Bank any Machine on site without a Valid and appropriate CPCS Card.
- Ride as a passenger on any plant or machine whilst on site.
- Climb onto scaffolding
- Leave tools or materials lying about on the ground creating trip hazards or at heights where they can fall and cause injury.
- Enter an excavation over 1.2m unless it is shored and battered to a safe slope.
- Consume or possess alcohol, drugs or any other intoxicants on site or be under their influence.
- Engage in any operation that may risk harm to yourself or others.
ACCIDENT PREVENTION
All MPL Site Services Ltd Operatives are reminded of the importance of safety and risk avoidance whilst working on site. General operatives should not use
power tools unless they have received formal training and have a certificate to prove that they have undergone the training and to prove their competence.
DECLARATION
I confirm that I understand the information on this form and agree that I will not attempt tasks that I am not qualified or competent to undertake.

- I confirm that do not have a language barrier that will prevent me understanding a written or verbal safety induction.
